This template features a logo, company name and address to specify at the top. Just below header, you can specify To and Ship To information. After that you can specify details of invoices over-due. Then a strict message to convince your client about the seriousness of this notice. Just before closing it, specify summary of credit and total sum of past due payments.
At the bottom specify mode of payment and a signature for the receiving person to ensure that notice has been received by the intended person.
